Jeff Garzik cff2680643 [SCSI] gdth: convert to PCI hotplug API 
- remove PCI device sort, which greatly simplifies PCI probe,
  permitting direct, per-HBA function calls rather than an indirect
  route to the same end result.

- remove need for pcistr[]
